ALL SERVICE REQUESTS ARE UNIQUE. AS SUCH EACH UPGRADE OR DISCONNECT/RECONNECT WILL BE SUBJECT TO AN ON SITE INSPECTION TO DETERMINE IF ADDITIONAL UPGRADES OR EQUIPMENT IS NEEDED TO COMPLETE THE MEMBERS REQUEST. IF ADDITIONAL UPGRADES OR EQUIPMENT ARE NEEDED, A COST ESTIMATE WILL BE GENERATED AND PROVIDED TO THE MEMBER. PAYMENT IN FULL IS REQUIRED PRIOR TO CONSTRUCTION SCHEDULING WITH GCEC LINE CREWS.

I HEREBY REQUEST AN ELECTRICAL SERVICE PANEL UPGRADE OR SERVICE DISCONNECT/RECONNECT AT THE ABOVE-REFERENCED LOCATION. I ACKNOWLEDGE THAT GCEC WORKLOADS MAY VARY; THEREFORE, THIS SERVICE REQUEST WILL BE SCHEDULED ACCORDINGLY AND MAY NOT BE COMPLETED ON THE DAY REQUESTED. I FURTHER UNDERSTAND PAYMENT MAY BE REQUIRED PRIOR TO THE COMMENCEMENT OF WORK. ALL SERVICE UPGRADES MUST COMPLY WITH CURRENT CONSTRUCTION AND SAFETY CODES AND ARE SUBJECT TO THE COOPERATIVE’S POLICIES, SPECIFICATIONS AND APPLICABLE CHARGES.
